Title: Community Justice worker (Service Navigation)

The John Howard Society is a criminal justice agency that provides services in the areas of prevention, intervention, and reintegration. Our primary focus is to reduce the impact of crime and its causes through the delivery of relevant programs.
Position Summary:The primary function of this position is to provide a streamlined, single point of entry for case management and service navigation, with a focus on children, youth, and young adults (ages 6–26) who are at heightened risk of involvement in, or victimization by, crime. This includes individuals who identify as Indigenous, Black, or racialized; members of 2SLGBRQ+ communities; newcomers; individuals living with disabilities; residents experiencing low income; and women. Service Navigation plays a vital role in strengthening protective factors among residents and enhancing overall community safety. Through this program, individuals will be connected with service navigators who offer individualized, one-on-one case management. This support includes intake and needs assessments, goal planning, and referrals tailored to the individual's unique circumstances. Following a referral, the service navigator will meet with the individual to build rapport and establish trust. Services will be delivered both in-person and virtually, ensuring accessibility and flexibility in meeting client needs.
Duties and Responsibilities- Work collaboratively with the Youth Services Manager to ensure all programs adhere to contractual obligations, service standards, and organizational expectations.
- Deliver programming to youth participating in the Service Navigation Program in both in-person and virtual formats, including daytime, evening, and weekend hours as required.
- Support ambassador coordination efforts, including scheduling, communication, and facilitation of ambassador meetings, ensuring alignment with program goals and community engagement initiatives.
- Assist with program reporting by collecting, organizing, and maintaining accurate data, contributing to internal and external reporting requirements in a timely manner.
- Conduct outreach to community partners, schools, and youth-serving organizations to promote program awareness, strengthen partnerships, and enhance service accessibility.
- Complete all required documentation accurately and within established timelines, ensuring compliance with organizational and funder requirements.
- Utilize the APIC model and relevant assessment tools to develop individualized care plans and targeted outreach strategies for clients.
- Create and maintain a safe, inclusive, and non-judgmental environment that supports youth engagement and well-being.
- Develop, adapt, and distribute educational and program materials tailored to program objectives and participant needs.
- Collaborate and liaise with school personnel, community agencies, and stakeholders across York Region to support coordinated service delivery.
- Monitor and evaluate program effectiveness, contributing to continuous quality improvement and ensuring services meet established outcomes.
- Adhere to all program standards, organizational policies, and directives, maintaining professionalism in all aspects of service delivery.
- Maintain appropriate professional boundaries and uphold strict confidentiality in all interactions with clients, partners, and colleagues.
- Attend and actively participate in team meetings, ambassador meetings, and organizational gatherings to support collaboration and program alignment.
