Horizon Health Network (Horizon) is seeking a strategic, motivated, and agile individual to join our team as a Regional Director of Population Health. The ideal candidate will be a senior expert on population health, providing expertise and strategic support regionally on population health initiatives, system improvements and identifying and addressing health disparities within a large organization.

Who you are
You hold a master’s degree in health or social science discipline. A concentration of study or advanced degree on population health and/or social determinants of health is considered an asset for this position. You have the ability to work with minimal supervision and possess strong organizational and time management skills. You have 7 to 10 years progressive leadership experience with experience in human resource and financial management, problem solving and critical thinking. You have additional training in project management and organizational change/improvement and trauma informed care (an asset). Using a Two-Eyed seeing approach, you foster an environment that supports traditional Indigenous ways of knowing & healing practices with evidenced based quality care. You also possess additional experience leading collaborative initiatives in a clinical setting (asset). You have demonstrated program and project leadership skills, including expertise in planning, implementation, and evaluation of programs.
