Position SummaryToriven™ is seeking an experienced Registered Practical Nurse (RPN) to support clinical intake, patient navigation, and service coordination within a virtual-first multidisciplinary healthcare environment serving children (ages 4+), adolescents, adults, and families across Ontario.
The RPN Intake Officer supports patients and families through the intake and onboarding process by helping them understand available services, supporting appropriate intake routing, and assisting with navigation across psychology, psychotherapy, speech-language pathology, learning support, and multidisciplinary care pathways.
This role is designed for an RPN with strong communication skills, emotional maturity, and experience working within mental health, child and youth, community, or multidisciplinary healthcare environments.
This is a non-treatment and non-therapy role focused on intake support, service navigation, patient communication, and operational intake coordination. The role does not provide psychotherapy, counselling, formal psychological diagnosis, crisis intervention services, medication administration, or ongoing case management.
Toriven™ is not a crisis service. Where urgent or emergency concerns arise, the RPN Intake Officer is expected to follow established escalation and emergency protocols appropriately.
Occasional in-person meetings, training sessions, or organizational gatherings may be required.
What You Will DoClinical Intake & Service Navigation- Conduct intake and service navigation conversations with patients, caregivers, and families in a calm, professional, and supportive manner
- Review intake forms and presenting concerns to support appropriate intake routing and service pathway coordination
- Help patients and families understand available services, care pathways, and differences between programs using clear and non-technical language
- Support operational matching of patients to appropriate clinicians and services based on presenting concerns, age group, and intake pathways
- Collaborate with clinicians and leadership regarding unclear, sensitive, or complex intake situations where additional review may be required
- Support a compassionate, organized, and patient-centered intake experience across all service lines
Patient Communication & Intake Support- Respond to patient and family questions related to services, intake processes, scheduling, and onboarding
- Support continuity across intake and onboarding workflows in collaboration with intake administration staff
- Assist with intake follow-up and coordination activities where required
- Document intake interactions and coordination activities appropriately within the EMR system
- Maintain professionalism, confidentiality, and strong communication standards in all patient interactions
Escalation & Safety Awareness- Recognize situations that may require escalation to clinicians, leadership, emergency services, or crisis resources in accordance with established protocols
- Remain calm and professional during emotionally sensitive conversations
- Support safe and appropriate patient navigation within established organizational and professional boundaries
