- Most positions Monday to Friday daytime. Some require a mix of days and evenings.

The PositionThe Service Coordinator will be a part our rural program that provides supports for adults with intellectual disabilities. The Service Coordinator will guarantee the availability of sufficient staffing and resources for the smooth execution of program activities, as well as integrate and advance supported individual’s files and support plans. This multifaceted role involves administrative tasks such as coaching, supervising, and guiding Direct Support Professionals (DSPs), in addition to providing direct assistance and case management for individuals receiving support.
What you will do- Coordinate scheduling for program activities and Direct Support Professionals, reviewing shift reports and approving timesheets.
- Anticipate decreased staffing levels or increased program needs, notifying the Program Manager and participating in the hiring process to fill needs.
- Complete and review various reports and documentation, which may include support profiles, Circle of Care Plans, Incident Reports, monthly reports, invoices, leases, housing benefits, hamper applications, etc.
- Handle financial matters both related to individuals receiving supports and employees, which may include financial plans, budgeting, authorizing program spending, and vehicle mileage submissions.
- Ensure compliance with, and gathering necessary feedback on, all legal and organizational requirements, policies, and standards.
- Help create, build, and maintain valued social roles for individuals receiving support.
- Participate in crisis support interventions, sending off incident reports in a timely manner, and responding effectively to individuals receiving support’s needs during emergencies.
- Flag concerns related to individuals receiving supports in shift reports and initiate appropriate action, collaborating with the team to address challenges proactively.
- Collaborate with Coordinators, Managers, and external stakeholders, such as CSWs to coordinate and implement care and support plans, facilitating effective teamwork and communication.
- Attend meetings and appointments as necessary for individual’s support, including CLPS, Court, Behavioural Specialists, CFS and team meetings.
- Organize team meetings and frequent one-on-one meetings with individuals receiving support to review progress, address concerns, and plan next steps.
- Provide direct support shift coverage when needed to support program activities, or as is required to meet funding mandates.
- Ensure basic needs of individuals receiving supports are met, including housing arrangements, by coordinating with relevant agencies and service providers.
- Provide safe and culturally relevant supports, ensuring that the supports offered respects and aligns with the values, traditions, and needs of the individual.
