Reporting to the Program and Resource Manager, you will be responsible for overseeing the overall success and health of Easter Seals Alberta
’s programs for the Medicine Hat region and you must be located in the area as you may be required to do outreach within the community. This position requires a strong background in delivering program excellence. The ideal candidate will have strong interpersonal and communication skills, along with extensive knowledge of community resources and needs based assessment tools.
Responsibilities:As part of our team, you will play a critical role in empowering individuals and families through empathetic support, collaborative partnerships, and innovative service delivery.
Client Support & Service Navigation- Provide clear, accurate, and accessible information on disability supports and services, including Persons with Developmental Disabilities (PDD), Family Managed Services (FMS), and Family Supports for Children with Disabilities (FSCD) agreements.
- Support individuals, families, and professionals in navigating systems, accessing resources, and coordinating services to meet their unique needs.
- Guide clients through needs-based assessments and co-create personalized service plans that promote independence, inclusion, and wellbeing.
- Connect clients to community and provincial resources, including financial supports, adaptive equipment, mental health services, and other disability-related programs.
- Assist clients in accessing and using digital tools (e.g., OCR system, Alberta.ca accounts, digital signatures) to support efficient service access.
Capacity Building & Education- Empower clients and families to confidently manage their supports by providing coaching on employer responsibilities, including hiring, training, and managing staff.
- Offer guidance on financial reporting and compliance requirements for FMS and FSCD agreements.
- Deliver engaging learning opportunities for clients, families, and professionals on topics such as FSCD and PDD programs, employer responsibilities, Financial Literacy, and best practices.
Program Delivery & Continuous Improvement- Support the development and implementation of key planning tools such as Individual Support Plans (ISPs), Transition to Adulthood Plans (TAP), Behavioural Support Plans (BSPs), and related processes.
- Monitor program performance, track outcomes, and gather client feedback to identify trends, challenges, and opportunities for improvement.
- Translate insights and data into actionable recommendations that enhance service quality and client outcomes.
- Develop and implement processes that improve workflow efficiency and strengthen service delivery.
