Job Summary and RequirementsThis is a until –filled competition. Who we areThe Government of Yukon's Continuing Care division is known for its exceptional standards, providing clients with the highest level of care and a healthy workplace for staff. The services offered are among the best in Canada, and Yukon’s long-term care homes ranked second in a recent report issued by the National Institute on Ageing. Continuing Care’s mission is to provide comprehensive care and services, including long-term care, home care, and regional therapy services, to the people of the Yukon. Watson Lake Home Care provides client-centred services in client’s homes to support them as they age in place.
Your roleResponsible for the professional nursing care for Home Care clients in Watson Lake, the Home Care Nurse-Home Support Supervisor coordinates client care and collaborates with other Home Care staff (ie. Physiotherapists, Occupational Therapists) and community partners, including Liard First Nation. This position involves care coordination for Home Care clients in Watson Lake and surrounding area, including long-term care admissions and hospital discharges. This role requires a Registered Nurse with excellent assessment skills and a passion for community practice. As a supervisor to full-time and on-call home support workers, this position is responsible for recruiting, training, supporting and scheduling staff as they work in clients’ homes as part of the home support program.
